Probably it happened due to a weak electrical contact. Conversion bulbs very often require a good start pulse and any weak contact (usally the bulb not screwed in fully) inhibits bulb start up.

and if screwing it in tighter dont do it, pull that tab up. basically the same thing but some times the threads bottom out before the bulb does
